　　 本劇根據真人真事改編而成，劇本來自原作者木籐亞也(附注一)生前的同名日記小說《一公升眼淚》，本劇描述了現實中14歲就患脊髓小腦變性症(附注二)，到後來不能說不能動的木籐亞也的前半生。劇中錦戶亮扮演的麻生遙斗陪伴著池內亞也慢慢走過，實際上是根據木籐亞也的母親木籐潮香女士，因女兒生前希望結婚談戀愛卻未能如願要求劇組添加的架空人物。
　　 今年15歲的池內亞也(澤尻繪里香飾)出生於一個平凡而幸福的家庭，一家六口每天的生活雖然平淡卻很幸福快樂。從小就是好學生的亞也考上了自己心儀的高中，原以為從此光輝燦爛快樂的人生正等著自己，沒想到卻患上了不治之症─脊髓小腦變性症。慢慢地，亞也的身體機能開始不受控制，走路無法保持平衡，經常跌倒，甚至不能好好寫字，看著這麼沒用的自己，亞也哭了一次又一次，當她哭著問醫生“為什麼病魔會選上我？”的時候，更是讓所有觀眾潸然淚下。

　　 在家人，以及喜歡的男生麻生遙斗(錦戶亮飾)的支持下，亞也下定決心努力無悔地度過每一天，不給自己的青春留下一絲遺憾。

　　 另外，本劇的目的並不是讓觀眾一起哭泣流下眼淚，而是希望通過亞也，讓觀眾從中得到勇氣，努力生存下去，學會珍惜生命。

　　 附注一：原作者木藤亞也(1962～1988年)14歲患上不治之症“脊髓小腦變性症”，身體機能逐漸開始衰退，到18歲家人才在萬不得已的情況下告訴她真相。亞也24歲的時候，即去世前一年，她母親整理的亞也的日記本《一公升眼淚》正式出書成冊。

　　 附注二：脊髓小腦變性症是以運動失調為主要症狀，病理學上是以小腦及其傳入、傳出途徑的變性為主體的疾病，臨床上是以肢體共濟失調和构音障礙為主要特征。大量臨床資料報告研究表明：小腦萎縮的大多數患者是屬於遺傳性的，且病情呈慢性、進展性惡化，若得不到有效的控制，很快就會危及生命。所以，一旦發現應及早用藥治療，有效地控制病情、改善原有的症狀、提高生活質量、延緩生命。但卻沒有徹底根治的辦法，屬於不治之症
